Fee Schedule
Up to age 18 $15 monthly, 19-35 is $40 per monthly, 36-50 is $60 monthly, 51-64 is $100 monthly, and 65 and up is $125 monthly. after the monthly fee, everything else (joint injections, etc) is free. So it is like a membership where monthly fees take care of everything, whether they see me twice a year or twice a month.
